1. Provides individualized attention and nursing care in accordance with resident Care Plan which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating, etc.
2. Responsible for observing and promptly reporting any changes in resident conditions to Charge Nurse/Nurse Manager (example: level of activity, eating habits, behaviors, and physical, mental and emotional status).
3. Performs tasks assigned by Charge Nurse/Nurse Manager including vital signs, residents’ weights, applying non-prescription creams/ointments, collecting specimens, etc.
4. Escorts residents to dining room, assists with feeding, and documents meal consumption.
5. Passes supplements, snacks, and fluids to residents per protocol and schedule and documents consumption.
6. Provides care that maintains each resident’s skin integrity in order to reduce the risk of pressure ulcers, skin tears, and other damage by changing incontinent residents per protocol, turning and repositioning immobile residents per protocol, and applying moisturizers to fragile skin as needed.
7. Maintains clean, neat and organized work environment.
8. Protects the personal belongings of each resident including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing and memorabilia. Promptly reports missing items to Charge Nurse/Nurse Manager per protocol.
9. Answers resident call lights promptly and courteously.

Job Description:

General Description: Under the direction of the Executive Director, provides clinical services in areas of nutritional documentation in accordance with current applicable federal, state, and local standards, guidelines and regulations and established policies and procedures, and provides oversight of planning, managing, and implementing of Food & Nutrition Services activities. Demonstrates ability to identify / respond appropriately to potential behavioral outbursts. In short-term absence of a Food & Nutrition Service Director, the Facility Registered Dietician will direct the Food & Nutrition Services Department, its programs and activities.

Essential Duties:
1. Verifies menu is served as written, including adherence to recipes, correct portion sizes, and therapeutic diets.
2. Verifies proper methods of food preparation are utilized to preserve nutrient content.
3. Makes meal rounds and verifies that new admissions are visited.
4. Participates in regularly scheduled patient care conferences and follows through on patient residents needs.
5. Demonstrates thorough knowledge of all Nutritional Services Department jobs and assists when necessary to promote smooth operation.
6. May need to assume responsibility for some cooking shifts depending on Facility size or available labor.
7. Monitors weekly costs associated with the Food & Nutrition Services Department and maintains accurate records / invoicing in order to report month end cost statistics and monitor year-to-date expenses.(in absence of Director of Food & Nutrition Services)
8. Completes nutritional history for new admissions, annual and significant changes, visits new residents for diet history, reviews medical record for pertinent nutritional information, records necessary information, notifies kitchen of likes / dislikes, beverage preference, and food allergies, and diabetic meal pattern, if needed.
9. Participates in QIP.

11. Participates in discharge planning, development, and implementation of patient Care Plan, patient assessments, etc.
12. Interviews patients and/or family members to obtain diet history.

14. Meets with administration, medical staff, and nursing staff, as well as other related departments in planning food service programs and activities.
15. Develops and maintains nutritional services objectives and standards.
16. Assists Director of Food & Nutrition Services in implementing and coordinating department policies and procedures.
17. Develops and participates in programs designed for in-service education, on the job training, and orientation classes for newly assigned department personnel.
18. Assists in the recruitment, selection, and training of a competent Director of Food & Nutrition Services as well as aids in the professional growth of the Director of Food & Nutrition Services.
19. Involves the resident / family in planning objectives and goals for the resident.
20. Assists in determining departmental staffing, evaluating employee performance, and making recommendations to the Executive Director concerning wage and salary adjustments, hiring, etc.
21. Inspects the Food Services area and practices for compliance with current applicable state and federal regulations.
22. Serves on various committees of the Facility as appointed by the Executive Director.
23. Conducts weekly weight committee meetings in conjunction with the Registered Dietician (RD).
